A cruise shows you the whole fiord from a deck, while a kayak puts you at the waterline beneath the cliffs. Here is how the trips differ in hours, age limits, weather risk and cost, and how to combine them.

Main facts
A cruise and a kayak trip share a fiord and little else. Milford Sound stretches 16 kilometres to the open sea, and a cruise runs the length of it, with Southern Discoveries saying every cruise goes the full length of the fiord, weather permitting.7,8 A kayak trip covers a fraction of that distance. The Sunriser Classic is 10km and the operator notes it does not go the length of the fiord to the sea.1,2
What you trade for distance is closeness. DOC describes the fiord as very steep-sided and busy with boat traffic, so a paddler shares the water with the cruise boats.9 The kayak operator argues that its sea kayaks go places the cruise boats cannot, with less noise and fewer crowds.10 That is a sales line from a tour operator, but it matches the physical difference between a deck and a seat just above the water.
For a Milford Sound cruise or kayak decision, choose by what you want to remember. If you want the waterfalls, the Tasman Sea entrance and a guide commentary without effort, take the cruise. If you want the sound of your own paddle and a close look at the rock, take the kayak. You can do both in the same stay, and the later sections show how the trips can be combined without turning into a single exhausting day.

Operator prices are on their own sites; the overnight listings start from $393 and $440 and are from prices, not exact quotes.
The operator that runs sea kayak tours on Milford Sound offers several trips with different effort. The Sunriser Classic is 10km with 3-3.5 hours of paddling, departs Deepwater Basin and is for ages 14 and over.1,2 The Milford Cruiser is the shortest kayaking trip on offer, and it suits people happy to spend up to 2 hours paddling and not going the length of the fiord to the sea.3,15
The Milford Cruiser leaves time for the rest of the day. The operator says it suits visitors who are short on time and have room to fit in a boat cruise or walk on the same day.13 A longer option, the Day Safari from Te Anau, adds return transport from Te Anau to the Sunriser Classic and lasts 9 - 11 Hours.16,17
Groups are small, usually six to eight paddlers with a local guide.18 You cannot rent a kayak to explore Milford Sound alone, because the operator's concession does not allow rentals.19 DOC notes that sea kayaking around Fiordland is challenging to reach but highly rewarding for skilled and adventurous paddlers, and a guided group is how visitors paddle there.20 Prices are on the operator's own site.
The age rules are strict. The operator requires a minimum of 8 years and 25kg, with 14 on the Sunriser Classic and the Paddle and Walk, and 16 on water taxi assisted trips.21 Anyone under 16 must be accompanied by a parent, caregiver or guardian on all of its trips.25 On the Milford Mariner overnight boat, kayaking is for guests aged 10 years and over, with an adult for children under the age of 16.22,26
Water and weather add their own risks. DOC says Fiordland waters can be very cold because of their large size, that the lakes and fiords are exposed, and that the wind usually strengthens in the afternoon.27,28 All of the operator's kayak trips are on the sea and can be affected by sea conditions, so a sea sickness remedy is recommended if you are prone to it.29
Sandflies are prominent in all areas of Fiordland, so DOC advises insect repellent.30 Any vessel entering the Fiordland Marine Area, even a kayak, must hold a Clean Vessel Pass, so ask your operator about it.31 Wildlife sightings from a kayak cannot be guaranteed, since many species are rare and endangered.32 Tell your guide about injuries or medical conditions before you go.

The overnight option
The easiest way to combine a cruise and a kayak is an overnight boat. The Milford Mariner anchors in Harrison Cove, where guests can paddle a kayak, take a tender boat excursion or swim, and it carries 20 kayaks and 2 tender craft.35,36 The listing for the Milford Sound overnight cruise with water activities includes optional kayaking of about 1 hour and optional swimming.
Kayaking from the boat is weather-dependent, and RealNZ says all activities depend on conditions.22 That is the difference from a dedicated paddle: the kayak is an option on a cruise, not the point of it. The listing runs 17 hours, starts from $393 and allows free cancellation up to 14 days before.
If you prefer separate days, The operator suggests the shorter Milford Cruiser paddle alongside a boat cruise or walk the same day.13 A coach and cruise day from Queenstown lasts 12 hours, so pairing it with a paddle is possible only if you are already staying near the fiord. You can also base yourself in Te Anau and paddle on the Day Safari instead.16

A paddle and cruise stay
Day before
DOC says Te Anau is the closest town to Milford Sound, a 2 hours drive away.11
Early start
The paddle is 10km with 3-3.5 hours of paddling from Deepwater Basin.1
Mid-morning
On fine summer days a strong breeze develops mid to late morning.12
Afternoon
The shorter Milford Cruiser leaves room for a boat cruise or walk.13
Next day
The overnight cruise leaves Manapouri at 12.00pm, with kayaking weather permitting.14,5
Doubtful Sound is the quieter place to paddle. DOC describes it as quiet, wild, isolated and beautiful, and notes that kayaks can be barged across Lake Manapouri to West Arm and driven over the Wilmot Pass.24 That route suits self-supported expeditions, while a boat-based paddle needs no barge.
On RealNZ's Doubtful Sound overnight cruise, guests can take a guided kayaking or tender boat excursion after anchoring in a sheltered cove, all weather-dependent, and children must be at least 10 years old to kayak.5,23 The listing for the overnight cruise from Manapouri includes optional kayaking and swimming, lasts 23 hours and starts from $440.
Allow for lake weather on the way in: DOC warns that Lake Manapouri can quickly become rough and choppy, particularly in the afternoons.37 The overnight departs at 12.00pm and returns at 12.00pm the following day, so the paddle fits into the evening or the morning of a trip that keeps you in the fiord for the night.14 The kayak is an option, so ask whether a Doubtful Sound overnight kayak is on the boat you book.

Milford Sound/Piopiotahi is a very steep-sided fiord with striking beauty, but busy with boat traffic. The southern side is more sheltered from the prevailing westerly winds, in particular the afternoon day breeze which is common during summer.
Before you paddle
Wind decides kayaking. The operator says strong winds can cause trips to be cancelled from time to time, while rain is not a reason to stop and all trips proceed as normal.6 On fine summer days a strong day breeze develops mid to late morning, which is why some of its trips start early.12 DOC says the southern side of Milford Sound is more sheltered from the prevailing westerly winds.9
Season matters too. The operator describes winter, April to October, as bringing more settled weather, very calm water and often no sandflies at all, and summer, November to March, as long daylight hours with sunrise and twilight trips.38,39 Plan the paddle for the calm end of the year if you are nervous on the water.
Check the terms before you book. The operator gives a full refund if you cancel at least 48 hours in advance.34 The overnight listings allow free cancellation up to 14 days before, and the Manapouri kayak tour with a water taxi allows it up to 24 hours before. Book the weather-sensitive paddle early in your stay, so a cancelled day can be rebooked.
